John's transition story

John is an autistic man who has a learning disability and bilateral hearing loss. He came to live in a bespoke self-contained flat at Mount View House, after a structured transition.
John’s transition to Mount View House
Before coming to Mount View House, John lived in another service with a different care provider. His social care team identified that he would benefit from living in a smaller home where he could have his own space. To meet John’s needs, Priory developed a bespoke self-contained flat for him on the grounds of Mount View House.
Whilst the construction took place, the team at Mount View House developed a structured transition plan for John. He came to visit to watch the build of his new home and develop relationships with his new support team and fellow residents. During this time, the staff team and two residents attended Makaton sessions with John’s previous support team. This helped them to develop their understanding and ability to communicate with John.
The teams at John’s previous home and Mount View House worked collaboratively at every stage of the transition. This helped ensure the right support was in place at his new home, and that things were progressing at a pace which suited his needs.
Support at Mount View House
The team at Mount View House worked with John's family and other professionals to develop person-centred support plans. He benefits from a robust, low arousal environment within his own flat, in a semi-rural setting. This means he is able to enjoy a peaceful life with minimal disturbances. He also receives input from the service’s positive behaviour support (PBS) practitioner.

This included implementing measures such seeking out medication changes from healthcare professionals and adjusting handover procedures. The team also hosted staff huddles, where they could exchange information about John's support more effectively between themselves.
John’s positive outcomes
With consistent support, John has now settled into his home at Mount View House. He has built strong relationships with his core staff team, and is now able to communicate his wishes effectively. Incidents of concerning behaviours have reduced significantly, and John is able to enjoy an enhanced quality of life.
John has also advanced his tolerance of situations which previously would have been difficult for him. Previously, he could only accept support from a small number of individuals, and this did not include females. Now, John copes well with receiving support from many different staff members at Mount View House, including both males and females. The team are also now able to wear personal protective equipment around him, without this causing distress for John.
Now settled, John enjoys a wide variety of activities both within the home and out in the community. He also likes socialising with fellow residents, taking part in activities with them in the main home. In addition, his team support him to maintain his relationships with his family, who he enjoys spending time with.
Thanks to the improvements in his wellbeing, John's social care team no longer consider him high-risk for placement breakdown. Accordingly, they have removed him from their dynamic support register. The management team at Mount View House are so proud of his achievements and look forward to seeing what he achieves next.
