About this location
Clyfton Crescent is a supported living service, offering support for four individuals with a learning disability, who may also be autistic or have mental health needs.
The service is located in Immingham, a small town located on the south-west bank of the Humber estuary. The home is situated in a quiet cul-de-sac and is a 10-minute walk from local shops. We form part of Priory Supported Living Lincolnshire, fall within the North East Lincolnshire local authority area and our properties are managed by Inclusion Housing provider.
At Clyfton Crescent we are committed to reflecting the guidance of The Reach Standards in everything we do. This means we work with our tenants to ensure they receive the right support to help them achieve their goals and reach their full potential. Our specialist support enables our tenants to live independently and make their own choices on every aspect of their lives, with help from our staff.

About our service
Our facilities and environment
Clyfton Crescent is a semi-detached bungalow, located on a corner plot with surrounding gardens to the front and back of the property. Communal areas include:
- A lounge
- Dining room
- A fully equipped utility room, complete with laundry facilities
- Garden
Our bedrooms
There are four bedrooms and tenants share a communal bathroom. The bedroom in the annexe has its own living room and small kitchen. All of our bedrooms are comfortable, welcoming and can be personalised to individual tastes. The rooms are all wheelchair accessible.

Our approach to support
Everybody who lives at Clyfton Crescent has their own person-centred support plan, which is co-produced with input from the individual, their family members, and professionals.
We aim to enable each tenant to make their own choices about the support they receive. This may include support in a number of areas, such as maintaining a tenancy, personal care, and support with household tasks such as budgeting, shopping and domestic tasks.
In addition, our team receive specialist staff training, bespoke to the needs of each individual we support. This includes positive behaviour support (PBS) training.
Activities
Everyone who lives at Clyfton Crescent is supported to pursue their own interests and hobbies, both in and outside the home, and are encouraged to take part in activities which are meaningful and fulfilling to them.
We focus on supporting tenants to grow their independence. For some people, this may mean developing their daily living skills within their home. For others, this could be accessing work or educational opportunities in the local community.
Pathways
Through our network of healthcare, residential and supported living facilities, we provide unique care pathways which enable the people we support to progress towards greater independence. Every individual we support is provided with a bespoke pathway plan, tailored according to their individual needs. We aim to ensure that each person is supported to live a fulfilling and active life in the community.
