Priory’s range of services enables us to provide unique and individual care pathways for the people we support. We can provide a seamless transition for each person as they progress between our higher and lower dependency services, with a goal of community living, or greater independence, wherever possible.
Our national network of services include both hospitals and community-based services. This means we're able to deliver care and support across a variety of different settings.

From the point of admission, we focus on providing a planned pathway towards greater independence, where each person has a completely bespoke transition plan, which considers their unique needs.
Priory has two assessment and transitions directors for complex autism. Their role is to carefully plan the transitions of autistic people who have complex needs. They support seamless care pathways from our hospital to community settings, as well as from services outside Priory. They work closely with our site teams and positive behaviour support (PBS) specialists, to ensure the right support is in place for each person.
Our aim is that people with complex needs are able to live the life that they choose in the community. By providing tailored support to each person in their pathway, we reduce the likelihood of placement breakdowns. The consistent support they receive at every stage enables them to progress at a pace that suits them.
